COMMUNITY HEALTH & WELLNESS PROGRAMS

We strive to promote health and wellness to all our Tribal families and the communities in which we live.

Programs and Services

Eligibility:
CTCLUSI Tribal members and CTCLUSI widows/widowers.

Services Provided

The Health Services Division is able to provide the following: limited OTC medications (some requiring a doctor’s written prescription) and certain safety items such as smoke alarms and bicycle helmets. Also available is the Ready for Baby Program, which provides an infant car seat and gift basket (includes educational materials, safety items and gifts for expectant Tribal parents).

Fitness Program Suspended: The Tribe has made the hard decision to suspend the Fitness Program until further notice. The majority of the funding comes from the General Fund and with the casinos closed for more than two months due to the Covid-19 Pandemic, we do not know at this time that there is enough money available for these reimbursements.

This suspension also includes the Nike shoe program. We apologize for any inconvenience this has caused. We will restart the program in the future when funding is available. Thank you for your understanding.

Eligibility:
CTCLUSI enrolled Tribal member, their spouse (must be legally married and currently residing with the Tribal member) and CTCLUSI widows/widowers.

Fitness Program Forms

Services provided:
Covers the cost of fitness-related activities, such as gym memberships, sports participation and limited weight-loss/management programs (does not cover medical weight-loss programs). There is also a benefit for equipment purchases of fitness equipment, subject to limits as identified in the program guidelines. Additionally, each adult participant can use $65 of their Fitness Program benefit to purchase one pair of Nike N7 shoes each year. At this time, Nike N7 shoes are not made in children’s sizes. See Program Flyer and Program Guidelines for more details.
