Eligibility:
CTCLUSI Tribal members and CTCLUSI widows/widowers residing in the 5-County Service Area.
Services Provided
The Health Services Division is able to provide the following: limited OTC medications (some requiring a doctor’s written prescription) and certain safety items such as smoke alarms and bicycle helmets. Also available is the Ready for Baby Program, which provides an infant car seat and gift basket (includes educational materials, safety items and gifts for expectant Tribal parents).
Eligibility:
CTCLUSI enrolled Tribal member, their spouse (must be legally married and currently residing with the Tribal member).
Services provided:
Covers the cost of fitness-related activities, such as gym memberships, sports participation and Club Fees as well as the purchases of fitness related equipment, (subject to limits as identified in the program guidelines).
Additionally, each adult participant can use $65 of their Fitness Program benefit to purchase one pair of Nike N7 shoes each year. At this time, Nike N7 shoes are not made in children’s sizes.
